Your guide to navigating NDIS forms and templates

Are you struggling to navigate all the NDIS forms and templates needed to get the most you can out of your NDIS plan? We’ll give you a brief rundown of some of the most common forms you may need to access over the course of your plan, including the NDIS access request form and the NDIS change of circumstances form.

It’s important to remember that when completing NDIS forms and templates that some forms and templates need to be completed a certain way, with specific information provided. As such, it’s often best to liaise with your LAC or contact the NDIA directly to request access to the forms and templates you may need throughout the duration of your plan. This article is designed to provide you with guidance to some NDIS forms and templates you may require, their purpose, and when to access them.

NDIS Access Request form

The NDIS Access Request form is the first of many NDIS forms you will encounter when requesting access to the NDIS. Now available for download from the website, its purpose is to determine the type of disability you have and whether you qualify for access to the NDIS.

At this point you will also need to consult with your GP or specialist to complete the supporting evidence form to ensure you provide the NDIA with as much information about your disability as possible.

NDIS Supporting Evidence form

When applying to access the NDIS, you will need to contact the NDIA for a NDIS Supporting Evidence form. The form will need to be completed by a GP, specialist or allied health professional with supporting evidence of your disability. This includes:

  • Diagnosis and treatment information, for example, a hospital discharge plan or mental health plan
  • Functional assessments like carer statements or assessments by allied health staff
  • Impact statements on how your disability affects your daily living

NDIS Consent form

The NDIS Consent form is a form that you will have to use fairly early on when dealing with NDIS. Essentially, the NDIS Consent form is your basic consent form, allowing the NDIA to share your information with other providers like Greater Support Coordination, medical professionals, allied health professionals, or anyone else who might need access to your information over the length of your NDIS plan.

NDIS Consent For A Third Party To Act On Participant’s Behalf form

The NDIS Consent For A Third Party To Act On Participant’s Behalf form is otherwise known as the NDIS Nominee form. The NDIS Nominee form comes into play when you are asking a third party to act on your behalf and liaise with your LAC. For instance, you may have had a change in circumstances and be unable to liaise with the NDIA yourself regarding your NDIS plan.

If you complete this form, the third party is allowed to make changes such as:

  • Submit a request to review NDIA decisions
  • Update your bank or personal details
  • Create service agreements and bookings with providers
  • Assist with other change of circumstances

NDIS Change Of Circumstances form
